1. Complete this Rebate Claim Form. Claims must be received by mail/email info@triadsupplyinc.com If the form is completed online, it will automatically be submitted.

2. Awards will be based on the points accumulated from purchases from Triad Supply, Inc.

3. Rebate Forms must be received by email. Please allow 5-7 business day to process.

4. This form must be completed by an authorized company representative to be valid.

5. Only Purchases of Triad Supply, Inc. products qualify for the Awards Program.

6. Any misrepresentations or fraudulent information disqualifies the claim.

7. Triad Supply, Inc. distributors, dealers, resellers and their employees are ineligible.

8. Only purchases by customers from a Triad Supply, Inc. reseller in the 50 United States are valid. Offer void where prohibited, taxed, or restricted by Federal, State, Provincial or local law.

9. Triad Supply, Inc. reserves the right to request additional information regarding this claim and the right to confirm identification.

10. Triad Supply, Inc. understands companies have different policies regarding rebates and awards. It is the sole responsibility of the claimant to adhere to his or her own company policy.

11. Any submission with an invalid or undeliverable address will be rejected.

12. Triad Supply, Inc. is not responsible for claims lost stolen damaged, illegible, misdirected or delayed in the mail. Please keep copies of all materials submitted. No claims against lost materials will be evaluated unless accompanied by proof of receipt of the original claim by Triad Supply, Inc.

13. The claimant waives any and all claims against Triad Supply, Inc. relating to this offer.

14. Any tax liability or tax reporting or compliance obligations associated with the accrual, use or redemption of any Triad Supply, Inc.. points is the sole responsibility of the redeeming company, its employees or agents.

15. Points may accumulate in a Triad Supply, Inc. account for up to 1 year.  However, the points may not be redeemed for cash.

16. Triad Supply, Inc. reserves the right to cancel or modify this program at any time.
